If you have made a booking, you will receive a link via SMS to an online Patient Information Form. If you are unable to complete the online form, please download, print, complete and bring the form to your appointment.
Care outside our normal operating hours
A doctor can be seen at the clinic outside our normal operating hours for minor emergencies. Please call 0416 584 840. An upfront fee is payable at the time of consultation. Click here for fees.
Do you have any symptoms of respiratory illness? i.e. fever, sore throat, cough, shortness of breath, runny nose, etc
Please notify us when booking your appointment. You may be requested to do a RAT test before visiting the practice.
